The SBAGNE Snowflake Network. 

Our goal is to be able to provide an uplifting care package for our members who are undergoing a procedure or recovery.  

To request a care package for a friend or family member, please complete this form.

Packages will be sent to a designated person who can deliver the package to the patient – we cannot mail to a hospital address. Other helpful suggestions are to send the patient/family gas cards, food delivery cards, or set up something like a Meal Train for them to provide support at home.
